This propane bottle is a twenty pound cylinder with a crushed foot ring. Notice how the foot ring itself is rusted severely. This rust probably contributed to the failure of the foot ring. This is probably the result of dropping the cylinder from a fairly decent height. A combination of weakened steel and blunt force caused this cylinder to be removed from service.
